How does a ferrule work?
• By encircling the dentine, it ensures that lateral forces on the post compress the dentine, rather than put it under tension. • This prevents fracture of the dentine (and of the post-cement, which is also weak in tension).
If you are unable to provide a ferrule, but the patient insists on a post crown, what should you do?
• Get written informed consent. Explain that the root may fracture, and ensure they understand this.
• Ensure they understand that subsequent corrective work will need to be paid for.
• Do not give an estimate of how long it may last.
• Record your discussions with the patient in their record (notes). This is most important - the consent form is not enough on its own to protect you from legal proceedings.
